What Is Vaginismus? How Pelvic Floor Physical Therapy Can Help

If you’ve ever felt pain, tightness, or fear when trying to insert a tampon, have a pelvic exam, or be intimate, you’re not alone. Many people experience these symptoms, and one possible reason is a condition called vaginismus.

The good news is that help is available. Pelvic health physical therapy can help you better understand your body, reduce pain, and feel more comfortable over time.

What Is Vaginismus?

Vaginismus is when the muscles around the opening of the vagina tighten without you trying to tighten them. This tightening can make it difficult or painful for anything to enter the vagina.

This can include:

  • Tampons
  • Pelvic exams
  • Vaginal dilators
  • Intimacy involving vaginal penetration

The muscles are not “choosing” to tighten. This happens automatically, much like blinking when something comes close to your eye. It is a protective response that your body cannot simply “turn off.”

What Does Vaginismus Feel Like?

Everyone’s experience is different, but common symptoms include:

  • Pain with attempted penetration
  • Burning, stinging, or sharp pain
  • Feeling like there is a wall or blockage
  • Tightness that makes insertion difficult or impossible
  • Fear or worry about pain before intimacy or a pelvic exam

Some people notice symptoms the very first time they try to use a tampon or become intimate. Others develop symptoms later in life.

What Causes Vaginismus?

There is not always one clear cause. Many different factors can play a role.

These may include:

  • Pain from a past injury or medical condition
  • Childbirth or surgery
  • Pelvic floor muscles that are overly tight
  • Stress or anxiety
  • A painful past experience
  • Fear of pain

Sometimes there is no obvious reason at all. No matter the cause, your symptoms are real, and you deserve care that takes them seriously.

How Can Pelvic Health Physical Therapy Help?

Pelvic health physical therapy focuses on helping your pelvic floor muscles work the way they should. The goal is not to force the muscles to relax, but to help them learn that they are safe.

Your physical therapist will create a treatment plan based on your comfort level and your goals.

Treatment may include:

  • Learning about how the pelvic floor works
  • Breathing and relaxation exercises
  • Gentle stretching and movement
  • Hands-on treatment, only if you are comfortable with it
  • Teaching the pelvic floor muscles how to relax and coordinate
  • Using vaginal dilators when appropriate and only with your permission
  • Exercises to improve movement of the hips, back, and core

An internal exam or treatment is never required. Your therapist will explain every step, answer your questions, and only do what you feel comfortable with.

How Long Does Recovery Take?

Recovery looks different for everyone.

Some people notice changes within a few visits, while others need more time. Progress depends on many things, including how long symptoms have been present and what may be contributing to them.

The most important thing is that healing is possible. Small improvements can add up over time.

You Are Not Alone

Many people with vaginismus feel embarrassed or think they should “just relax.” But vaginismus is a real medical condition, and it is not your fault.

With the right support, many people are able to reduce pain, feel more comfortable, and return to activities that once felt impossible.

When Should You Seek Help?

If pain or tightness is making it difficult to use tampons, have a pelvic exam, be intimate, or affecting your quality of life, it may be time to speak with a pelvic health physical therapist.

Getting help early can make a big difference.
